Work Permits (Summer Info)

Dear Students, 

If you would like to have a work permit issued over the summer, you will need to contact Ms. Christina Becerra at cbecerra@cv.k12.ca.us

  1. Get the Intent to Employ form: STATEMENT OF INTENT TO EMPLOY A MINOR AND REQUEST FOR A WORK PERMIT–CERTIFICATE OF AGE
  2. You will need to complete the “Minor’s Information” section and “School Information” section.
  3. Your parent or guardian needs to complete the section called: “To be filled in and signed by parent or legal guardian”
  4. Your employer needs to complete the section called: “To be filled in and signed by employer”
  5. When these sections are completed, you will need to email the completed for to cbecerra@cv.k12.ca.us and she will prepare and return the Work Permit in approximately 24 to 48 hours.
